Two of Newcastle United's loanees suffered EFL Cup second round exits in midweek.
On Wednesday night, Shrewsbury Town travelled to Wearside to face Sunderland, and striker Ivan Tony was named in their starting line-up for the fourth consecutive match.
With the game still goalless, Toney went closest for the League One side at the Stadium of Light but saw his 56th-minute effort saved by Black Cats keeper Jordan Pickford.
And Sunderland snatched the victory late on through Adnan Januzaj's eye-catching finish.
Toney played 79 minutes of the tie before being replaced by George Waring - his fifth appearance in total for Shrewsbury.
On Tuesday in the same competition, Macaulay Gillesphey and Carlisle United were dumped out of the cup following an extraordinary penalty shoot-out.
Trailing 1-0 against Championship side Derby County at the iPro Stadium, the Cumbrians levelled in added-on time through Mike Jones, and the sides were then unable to be separated during half-an-hour's extra-time.
So it was be decided from the spot, with Derby eventually triumphing 14-13.
Gillesphey, who played the full 120 minutes of the tie, converted his spot-kick in the shoot-out and was next in line to take a second effort, but wasn't needed for another go as teammate Joe McKee's miss proved decisive.
Elsewhere, young midfielder Liam Smith missed Crewe Alexandra's dramatic 4-3 extra-time defeat at Blackburn Rovers through injury.
